2006 FRINGE BENEFITS FACTS
Summary of Benefits
Miami-Dade County Public Schools with the combined efforts of its unions and associations provide all full-time employees with the following benefits.
LIFE INSURANCE: (Board paid)
Term life insurance in the amount of one time your annual salary. You may purchase additional (Optional) life insurance for yourself or life insurance for your eligible dependent(s). These additional life insurance premiums are paid through payroll deductions
STANDARD SHORT-TERM DISABILITY (STD) Board paid
STD will pay you 60% of your base salary, not to exceed $500 per week after you are disabled for 30 days. Upgrades are available whereby an employee may increase the benefit from 22 weeks to 24 weeks, reduce the elimination period and pay a higher weekly maximum. These upgrades would be paid through payroll deductions.
HEALTH INSURANCE: (Board Paid)
You may choose from one of the three plans, offered through United Healthcare, listed below:

In addition, the School Board also contributes a monthly amount toward each tier of dependent coverage. The per pay deduction rates in the 2006 Summary Guide and the Employee Online Benefits Notebook reflect the employee cost for dependent coverage.
OR
OPT-OUT PROVISION (Healthcare Declination) Board Paid
Employees may decline Board-Paid healthcare coverage, providing they agree to comply with provisions set forth in the Declination of Healthcare Affidavit. The employee must attest that they have healthcare coverage elsewhere. The Board will provide employees with $100 per month opt-out benefit, which will be used towards a flexible benefits package, i.e., 401(k) contribution, Flexible Spending Accounts (FSA), or cash.
FLEXIBLE BENEFITS: (Employee Paid)
You may purchase any of the additional flexible benefits listed below for yourself and your eligible dependents on a pre or post tax basis through payroll deductions.
