Title:               General Drug Test Authorization.

Description:    To authorize specimen collection in support of general drug testing. Specific use relates to authorization or exclusion of employment. Results are reported to the respective work location requesting the hiring action or compliance with existing contractual provisions.

Title:              Service Provider Input Document.

Description:    To fingerprint non-employees and provide them with an employee number.

Title:               ACES Input Document - Teachers Only.

Description:    To fingerprint non-employees and provide them with an employee number.

Title:               ACES Input Document - Support Personnel Only.

Description:    To fingerprint non-employees and provide them with an employee number.

Title:               Employee Name Change/Date of Birth Correction/Social Security Correction.

Description:    The purpose for this form is to allow Active Employees access to change their name and correct any discrepancies in their birthday or social security number only with valid documentation attached.

Title:              Current Substitute Change of Address Form.

Description:    To allow current Subtituess access to change their address in order to receive their W-2 form or any other information from the School Board.

Title:               Inactive Employee Change of Address Form.

Description:    To allow Inactive Employees access to change their address in order to receive their W-2 form, retirement information, payroll checks, and other information that is required to be sent to an inactive employee.

Title:               Letter of Resignation.

Description:    This form is to be used when an employee voluntarily resigns from the Miami-Dade County Public Schools.


Form Nº:         Ethrace

Title:               Ethnicity/Race Change Form.

Description:    In order to comply with federal reporting requirements, every school district in Florida is required to report to the Florida Department of Education each employee’s race and ethnicity on an annual basis. The Florida Department of Education does not report individual data to the federal government but does report the total number of educational staff in various categories in each school. Please complete the form below to update your information which will be added to your file.
